Clinical Nurse Manager, Patient Flow & Discharge

Mowlam Healthcare is recruiting a CNM, Patient Flow for Leopardstown Care Centre in Murphystown, Dublin. The role supports and deputizes for the ADON, Patient Flow to manage resident and patient referrals and admission planning.

The CNM will ensure safe discharge planning, act as case manager for DMR residents, and coordinate with hospitals, doctors, and discharge coordinators while upholding HIQA standards and organisation policies.
