Clinical Lead Nurse: Community Frailty & Virtual Ward

Hampshire and IOW trust is seeking a Band 6 clinical lead to join a fast-paced, dynamic team in the Southeast Division. You will lead clinical care in a virtual ward, focusing on preventing hospital admissions and supporting patients to remain at home during acute illness.

Experience in community care is desirable; full training and post-registration courses are available. Applicants must have the right to work in the UK; visa sponsorship is not offered for this post.
