OverviewWe are seeking a warm, confident, and clinically experienced leader to serve as our Clinical Director focused on client intake, assessments, and service onboarding. This role is ideal for a nurse, social worker, or clinician with hospice, assisted living, or home care experience who thrives at the intersection of clinical care, relationship-building, and consultative sales.
You will be the first clinical point of contact for prospective clients and families-guiding them through one of the most important decisions they will make. The ideal candidate is compassionate yet assertive, professional, detail-oriented, and comfortable discussing care plans, pricing, and securing service agreements.
Key Responsibilities- Serve as the clinical lead for all new client intake and onboarding
- Respond promptly to inbound inquiries from prospective clients and families
- Conduct consultative conversations to assess care needs and recommend appropriate services
- Perform in-home client assessments and develop initial care plans
- Confidently present service options, pricing, and secure client agreements/deposits
- Drive conversion of qualified leads into active clients
- Oversee the full onboarding process, including clinical documentation and intake paperwork
- Ensure compliance with all clinical and regulatory requirements during intake
- Case Management - responsible for all client's receiving quality care
- Maintain up-to-date care plans as client's conditions progress
- Collaborate with care coordination and scheduling teams to ensure smooth service start
- Provide early-stage follow-up to ensure client satisfaction and continuity of care
- Maintain accurate and timely records in CRM and clinical systems
- Clinical background required (RN, LPN, MSW, or related field strongly preferred)
- 3+ years of experience in home care, hospice, assisted living, or healthcare services
- Proven success in intake, admissions, or consultative sales within a healthcare setting
- Strong ability to build trust quickly with families in sensitive situations
- Comfortable discussing pricing, collecting deposits, and closing service agreements
- Experience completing clinical assessments and onboarding documentation
- Excellent communication, organization, and follow-through
- Tech-savvy with CRM or client management systems
- Valid driver's license and reliable transportation (company vehicle available for visits)
- Warm and empathetic, yet confident and decisive
- Professional and polished in both clinical and customer-facing settings
- Comfortable balancing clinical integrity with business goals
