Job Description
Clinical Coder & Summariser Administrator

To support the smooth and efficient running of the GPsurgery by accurately clinically coding and summarising patient medical recordsin line with practice protocols, NHS guidance, and information governancerequirements. The post holder will ensure patient records are maintained to ahigh standard to support safe clinical care, quality improvement, andcontractual requirements.

Working hours over a 5 day week - Monday to Friday with a finish time of 5pm each day.

Main duties of the job

The successful candidate will have excellent attention to detail, good organisational skills, and the ability to work both independently and as part of a team within a confidential healthcare environment.

About us

Ling House Medical Centre is a traditionally run GP Practice based in Keighley with Patient care at it's center. The Practice runs from two sites which are within easy walking distance from each other. We have facilities in place for easy access for disabled people and parking is available.

We are a well established teaching Practice with a large team of clinicians including GP's, Advanced Clinicians, Nurses, Health Care Assistants, Physiotherapists, Pharmacists and Social Prescribers who are supported by our experienced Reception, Administration and Management team.

Job responsibilities

Clinical Coding (training will be provided)

  • Accurately code and enter patient information onto the clinical system using appropriate clinical coding standards (e.g. SNOMED CT).
  • Review incoming correspondence including hospital letters, clinic notes, pathology reports, NHS email, and other clinical documentation.
  • Extract relevant clinical information and apply accurate codes to patient records.
  • Ensure coding is completed within agreed timescales and practice protocols.
  • Maintain consistency and accuracy of coded data to support clinical care, audits, QOF, enhanced services, and reporting requirements.
  • Identify and elevate any discrepancies, safeguarding concerns, or urgent clinical information to the appropriate clinician.
  • Scanning of documentation

Medical Records Summarising (training will be provided)

  • Summarise new and existing patient records onto the clinical system.
  • Review patient histories and extract relevant past medical history, medications, allergies, immunisations, and significant clinical events.
  • Ensure summaries are concise, accurate, and clinically relevant.
  • Prioritise workflow to ensure records are summarised promptly and efficiently.

Administrative Support

  • Support the management and maintenance of electronic patient records.
  • Assist with data quality work and practice audits where required.
  • Work collaboratively with clinicians, administrators, and management to improve record accuracy and workflow efficiency.
  • Participate in practice meetings, training, and development activities.
  • Maintain strict confidentiality and comply with GDPR, NHS confidentiality standards, and information governance policies.

Qualifications
  • GCSEs (or equivalent) at Grade C4 or above.
  • NVQ Level 2 or Level 3 in Business Administration, Health Administration, or equivalent qualification.
  • Clinical Coding qualification or accredited coding training.
  • AMSPAR qualification or equivalent healthcare administration qualification.
  • Further training in medical terminology or primary care administration.
Skills and Attributes
  • High level of accuracy and attention to detail.

  • Strong organisational and time management skills.
  • Professional, reliable, and adaptable approach.
  • Ability to work effectively as part of a multidisciplinary team.
Experience
  • Previous experience within a GP surgery, primary care, or healthcare administration environment.
  • Knowledge of medical terminology and clinical coding principles.
  • Experience using GP clinical system - SystmOne.
  • Excellent attention to detail and accuracy.
  • Ability to manage workload independently and prioritise tasks effectively.
  • Good communication and organisational skills.
  • Understanding of confidentiality and data protection requirements.
  • Experience with SNOMED CT coding.
  • Previous experience in clinical coding or medical records summarising.
  • Knowledge of QOF and primary care targets.
  • Clinical coding qualification or willingness to undertake training.
