Client Care Liaison

We are recruiting a Client Care Liaison to ensure every one of our clients receives the best person-centred, high quality care.

Guardian Angel Carers is a warm, professional home care agency supporting individuals and families across Wimbledon, Kingston and the surrounding areas. We help people remain independent at home, with compassionate home help and care at home tailored to their unique needs.

The Client Care Liaison will be responsible for:

• Ensuring care plans are accurate, up to date and reflective of clients' current needs
• Facilitating regular client reviews and risk assessments
• Accurate medication management and compliance
• Communicating effectively between the client, their family and wider multidisciplinary teams (MDT)

The Client Care Liaison will need to evidence:

• Previous experience of at least one year as a home care carer
• Previous experience of care plan writing and risk assessments
• Previous experience of medication and compliance procedures
• Awareness of confidentiality processes

To succeed in this role you will require:

• Level 3 Diploma in Lead Adult Care Worker (or working towards)
• Full UK driving licence and access to your own car

You will be able to demonstrate:

• Solid planning, scheduling and problem-solving skills
• Excellent communication and customer care skills
• Team player mentality
• Ability to quickly develop good rapport with people
• Flexible, enthusiastic and can-do attitude
• Good literacy and numeracy
• Organised and professional at all times
• Proficient use of computer, Microsoft office suite and databases
• Accurate data recording
