JobDescription

The role of the CarePlanner is to support the delivery of the highest quality caresupport services by carrying out a range of functions in the field,including the assessment and review of individual services and thepractical supervision, instruction and support of careworkers.

Working with Care Coordinators tosupport the planning of realistic, effective care worker rotas,with due regard for the particular needs of the individual and theparticular skills and attributes of the worker. Carrying outregular reviews of individual services, including repeated andrenewed needs and risk assessments. Participate as required inmulti-disciplinary review of service users' needs. Shadow new careworkers in the field to ensure their competence. Carry out theon-site observation and evaluation of care worker practice as partof routine supervision, in response to particular performanceconcerns, or as otherwise directed.

Senior CareAssistant are expected to spend the majority of their working timein the community carrying out their primary functions, but theirrole will typically also entail a certain amount of office-basedadministrativework.

Qualifications

Youwould need experience in carrying out initial risk and needsassessments in respect of individual service users, either in theservice user's home or, where required, in other settings such ashospitals or residential/nursing homes; Work in partnership withservice users, their family, carers, advocates, healthcareprofessionals and others to ensure the seamless delivery ofholistic packages of individual caresupport.
