Care Manager: Transition & Recovery for Young Adults

Bournemouth Borough Council is seeking a Care Manager for the Preparation for Adulthood Team within Adult Social Care. This 6-month fixed-term position involves working 37 hours weekly, collaborating with health and social care partners to support young adults in their recovery and independence.

The ideal candidate will have relevant experience, excellent communication and organisational skills, and a valid driving licence. The role includes conducting assessments, developing plans, and maintaining high safeguarding standards.
