Care Leavers Navigator - Pathways to Independence

Birmingham Children's Trust is seeking an experienced Social Worker / Personal Adviser to support children in care and care leavers aged 18-25. The role focuses on helping young people transition to adulthood, managing caseloads and coordinating services across accommodation, education, employment, finances and health.

You will complete, review Pathway Plans and provide direct work and practical support, ensuring young people have access to statutory entitlements and services.
