Care Coordinator

The Care Coordinator role at Agincare UK is responsible for matching carers to clients and ensuring the continuity of care across the South West region. It involves tasks related to new client starts, scheduling, safeguarding, and communication with various professionals. Main duties of the job

As a Care Coordinator, you will be responsible for matching our carers to our clients across the South West region. This includes supporting forward planning of scheduling, ensuring continuity of care, understanding safeguarding arrangements, and communicating with external sources such as next of kin, GPs, and other professionals. You will also be part of the on call rota system during evenings and weekends.

What is required from you?

• A minimum of 12 months on your right to work in the UK.
• A full UK driving licence and access to your own vehicle.
• Great level of attention to detail and well organised.
• Availability to work evenings and weekends as part of the on call rota.

A day in the life of a Care Coordinator

• You will support forward planning of scheduling and ensure it is consistently achieved to provide continuity of care.
• You will ensure all clients are safeguarded against the risk of abuse and respond appropriately to any allegations or complaints regarding service delivery.
• You will prioritise communication with external sources such as next of kin, GPs, nurses, physiotherapists, and any other professionals involved, keeping the Registered Manager updated as appropriate.
• You will ensure that all Care Worker holidays, sickness, unplanned absences, and emergency calls are documented electronically.
• You will be part of the on call rota system during evenings and weekends as and when required.
