We are currently recruiting a Care Coordinator to support children and adults with complex physical disabilities and life-limiting conditions across West London. This is a fantastic opportunity to join a compassionate, nurse-led team delivering high-quality care in clients' homes.

• Coordinate and oversee the mobilisation of new complex care packages

• Set up care plans, complete risk assessments, and ensure regulatory compliance

• Liaise with clients, families, healthcare professionals, and internal teams

• Ensure timely and seamless delivery of care services

• Maintain strong working relationships across the organisation

• Competitive salary: £30,000 - £35,000 per annum
