Main duties of the job

You'll be a core member of our Crisis Care team, delivering rapid, community-based support through Rapid Response, Home Intensive Treatment, and medical discharge pathways-helping prevent hospital admissions, support safe returns home, and empower young people to stay in their own homes.

Key Responsibilities

• Assessment & Intervention: Conduct psycho social and risk assessments, create tailored care plans, and deliver creative interventions to support mental well being.
• Multidisciplinary Collaboration: Work closely with CAMHS, acute and specialist hospitals, police, education, and social care to provide coordinated support.
• Crisis Management & De escalation: Use advanced de escalation techniques to manage acute distress and reduce risk.
• Community Based Support: Provide intensive home based care to prevent A&E visits and inpatient stays, boost family/community integration, and build resilience and life skills in CYP.
• Shared Caseloads: Manage a small caseload within a multidisciplinary team, offering mutual support and ensuring comprehensive care delivery.
• Operational Hours: This role includes evening and weekend shifts (8 am-10 pm, 7 days a week).

Person Specification Qualifications

• Relevant core professional qualification in mental health (Nurse, Social worker or Occupational Therapist)
• Qualifications in Psychological Therapies e.g. CBT / SFT / IPT
• Mentorship course ENB998 or equivalent

Registration/Accreditation

• NMC, SW England, UKCP, HCPC, BABCP

Knowledge, Skills and Ability

• Up to date knowledge of child mental health and relevant legislation.
• Understanding of evidence based practice and the ability to demonstrate how this influences clinical practice.
• The ability to assess, formulate and manage care appropriately, ensuring discharge is timely.
• Use assessment tools and outcome measures / ROMs effectively and in collaboration with the child, young person, and family.
• Function as part of a multi disciplinary team and liaise effectively with other agencies (e.g., education, GP's), maintaining positive working relationships.
• Reflect on and critically appraise own practice.
• Manage own caseload.
• Organise own time and diary.
• Follow policy and propose changes.
• Work independently with clinical and management supervision.
• Maintain clinical records, both written and electronic.
• Deliver training.
• Demonstrate therapeutic skills.
• IT competence.
• Knowledge and understanding of the research process.

Experience

• Proven experience of working effectively with people with mental health difficulties.
• Experience of assessing and managing risk, including safeguarding.
• Experience of developing and providing evidence based assessments, formulations, and care plans.
• Experience of liaising and working with a range of other relevant agencies and professionals.
• Experience of autonomously managing a caseload within a service.
• Experience of mentoring and supervising others.
• Understanding of the principles of Clinical Governance and a willingness to follow Trust Policy and Procedures.
• Community mental health experience.
