Post Summary- To be a proactive member of the pharmacy department, working collaboratively with our diverse team of pharmacy professionals, and to provide excellent pharmaceutical care to our patients.
- To act as Senior Pharmacist during each rotation which includes Acute and Integrated Medicine, Ambulatory and Same Day Emergency Care, Surgery and ITU, Women's Health and Children's Services (including Neonatal Medicine), Antimicrobial Stewardship, Anticoagulation & Stroke Prevention, Medicines Information, Clinical Trials and Governance.
- To ensure the seamless transfer of care of our patients between various care settings and ensuring that medicines reconciliation is performed accurately at all times.
- To mentor and undertake the educational and practice supervision of pharmacy team members during their various stages of professional development, including Band 6 and Pre-Registration Pharmacists, and to provide clinical support and training to any other relevant health care professionals.
- Ensure the delivery of a high quality, patient-orientated Pharmacy service in all areas of the Band 7 rotation. The areas for Band 7 rotation include: Acute and Integrated Medicine, Ambulatory and Same Day Emergency Care, Surgery and ITU, Women's Health and Children's Services (including Neonatal Medicine), Antimicrobial Stewardship, Anticoagulation & Stroke Prevention, Medicines Information, Clinical Trials and Governance.
- Deputise for the Specialist Lead Pharmacists whilst working in their rotation, attending committees and meetings when appropriate.
- Provide guidance on the safe and cost-effective use of medicines, and advice on the legal and procedural aspects of the prescribing and administration of medicines.
- Proactively support patient services, contributing in any activities related to the delivery of clinical pharmacy services to our patients.
- Provide information to nursing and medical staff on all aspects of drug usage and availability.
- Supervise Band 6 Pharmacists, Pharmacy Technicians, Pre-Registration Students, Pharmacy Assistants and Summer Placement Students, and to assist in their education and training.
- Participate in the continuing education and training programme for pharmacy staff, this may include the provision of lectures and tutorials.
- Assist in the training of nurses and other non-pharmacy staff in pharmacy related subjects.
- Lead on and support Audit, Quality Improvement and Service Transformation projects relating to medicines management, and contribute to the production of clinical guidelines and protocols.
- Counsel patients with regard to all aspects of the use of medicines supplied to them.
- Undertake Continuing Professional Development in accordance with Trust & General Pharmaceutical Council guidelines.
- Follow all established procedures and maintain all relevant records.
- Undertake appraisals of assigned pharmacy staff within the team and act as a Line Manager, when required.
- Contribute to the production of financial reports for the specialist clinical directorate.
- Participate in late duties and Saturday, Sunday and Bank Holiday duties as required and in the provision of cover for pharmacy services "out of hours" (i.e. Pharmacy On-call service)
- Carry out any other reasonable duties as requested by the Chief Pharmacist and Deputy Chief Pharmacist.
- UK Pharmacy Degree or equivalent.
- Full registration with the GPhC or undertaking pre-registration training due to be completed by the time of appointment.
- Prior pharmacy related work experience.
