Join us at HMP Lowdham Grange as a Band 7 Senior Primary Care and Senior Mental Health Nurse. This is an exciting opportunity offering a rate of £31 per hour with an ongoing contract. We are seeking individuals to fill these roles full-time, Monday to Friday. Responsibilities
  • Lead chronic disease management, providing direct care and guidance for conditions like diabetes, COPD, and hypertension.
  • Deliver advanced clinical assessments and health promotion emphasizing prevention and wellness.
  • Support audit processes, champion clinical governance, and drive continuous service improvements.
  • Mentor junior nurses and Healthcare Assistants, fostering learning and excellence.
  • Liaise with GPs, pharmacists, social care, and other PCN staff to ensure seamless continuity of care.
  • Conduct complex mental health assessments and lead crisis interventions and care planning.
  • Supervise and nurture development in junior nurses, students, and support workers, ensuring high standards of care.
  • Contribute to clinical governance, participate in incident reviews, and lead quality improvement initiatives.
  • Support Mental Health Act/Mental Capacity Act responsibilities, safeguarding procedures, and liaison with families/carers.
  • Chair multidisciplinary team or review meetings to facilitate comprehensive and collaborative care approaches.
