What you will do

• Deliver specialised physiotherapy interventions to patients within the Acute Frailty unit and supporting hospital areas.
• Complete comprehensive assessments and develop personalised treatment and rehabilitation plans.
• Work collaboratively with a multidisciplinary team to ensure coordinated and effective patient care.
• Educate patients and families on mobility strategies and rehabilitation techniques to promote independence.
• Contribute to service development by supporting innovative practices within the acute frailty care pathway.
