About the RoleYou will join the Assertive Outreach and Rehabilitation Team (AORT), a dedicated multi-disciplinary service supporting individuals with severe mental illness and complex needs such as substance misuse, high-risk behaviours, poor engagement, and social exclusion. Your role will focus on managing a caseload, conducting evidence-based assessments, and developing person-centred interventions. You will work collaboratively with colleagues, lead service improvements, and empower service users to achieve recovery goals, increase independence, and address their physical health needs.
Key ResponsibilitiesConduct person-centred assessments and create tailored, outcome-focused care plans.
Collaborate within the MDT (Multi-Disciplinary Team) to deliver integrated care and recovery strategies.
Provide supervision and mentorship to junior staff, fostering a supportive learning culture.
Complete risk assessments and implement effective risk management plans.
Build relationships with local services to promote user independence and continuity of care.
Maintain accurate, up-to-date records and documentation in line with service standards.
RequirementsRegistered Mental Health Nurse, Social Worker, AMHP, or Occupational Therapist (degree-level qualification).
Experience working in mental health settings within a multi-disciplinary team.
Skilled in managing complex cases and supporting challenging service users.
Strong leadership abilities and experience supervising staff.
Excellent knowledge of assessment, care planning, and risk management processes.
Desirable: Car owner and current driving licence.
