Job Summary

• Work within the Trust's CPA/Care Management framework, promoting high standards of patient care and communication.
• Act as a care co-ordinator for a defined caseload of clients with severe and enduring mental health problems.

What you will do

• Manage a flexible caseload that meets the unique needs of clients and their carers.
• Provide high standard individualised client care, assessing mental, emotional, and physical well-being.
• Act as a care coordinator to assess client needs and ensure care plans are implemented and updated regularly.
• Participate in crisis intervention measures and manage the admission, discharge, and transfer process of clients.
• Supervise therapeutic treatments, ensuring compliance with Trust Policy and NMC Standards.
• Engage in weekly clinical meetings, collaborate with in-patient facilities, and monitor the efficacy of care plans.
