Autonomous Advanced Clinical Practitioner - Home Visiting

Integrated Care 24 (IC24) is seeking an Advanced Clinical Practitioner for the Home Visiting team in Ashford and surrounding locations. You will assess and treat patients in their own homes, using advanced clinical reasoning to provide safe, effective care and appropriate escalation when needed.

You will work autonomously, managing a diverse caseload including urgent care, palliative and end of life needs, while collaborating with GPs and community teams to keep patients safely in their homes
