Ageing Well Care Navigator (Part-Time)

A healthcare provider in Havant is looking for an Ageing Well Care Co-ordinator to manage patient care coordination and support through health services. The ideal candidate will have at least 2 years of relevant healthcare experience and be able to develop individual health plans in partnership with patients. This role demands strong problem-solving skills and the ability to work collaboratively across various disciplines.
