Advanced Social Worker: Hospital Interface & Discharge Lead

Somerset Council in Taunton is seeking an experienced Advanced Practitioner Social Worker to lead the Hospital Interface Service within an acute hospital setting, delivering high quality adult social care leadership.

You will support adults with complex needs during admission, stay and discharge, champion Home First and person centred discharge planning, and work closely with health partners to reduce avoidable admissions.
