Internal only vacancy for staff employed at Bridgewater Community Healthcare NHS Foundation Trust and Warrington and Halton Teaching Hospitals NHS Foundation Trust.
Join the Warrington Urgent Community Response team as a senior clinical professional. You will provide an urgent rapid response to residents experiencing a health crisis and support them to remain at home where possible.
Job PurposeAct as an integrated health and social care team member, providing specialist treatment and advice at home to prevent unnecessary hospital admission. All referrals will be triaged immediately and responded to within two hours, with referrals from NWAS prioritised.
Responsibilities- Participate in and undertake triage to ensure smooth progress of patients through the system.
- Comprehensively assess patients with complex physical, mental and social care needs using advanced clinical examination skills, detailed history taking and biometric monitoring.
- Receive patients with undifferentiated and undiagnosed health care problems and initiate and conduct clinical investigations as appropriate.
- Analyse signs and symptoms, laboratory tests and other measures of function to formulate a diagnosis.
- Perform medication review and prescribe and change medications as required in accordance with national and local guidance.
- Exercise independent and delegated prescribing rights in accordance with current legal and ICB criteria.
- Develop short term individualised treatment and management plans using evidence based practice and monitor review care plans for up to 72 hours.
- Identify subtle changes in condition, take appropriate action, and plan preventative and anticipatory care needs.
- Act as an advocate for the patient, ensuring their individual needs, preferences and choices are considered.
- Collaborate with team members, promote professional development, and maintain accurate documentation in line with NMC guidance.
- Participate in clinical audit, research, and development of policies and guidelines.
- Maintain effective communication with primary, secondary, and social care partners and uphold confidentiality.
- First Level Registered Nurse or Allied Health Professional; current registration.
- Advanced Clinical Skills Modules at Master's Level (Level 7) in clinical examination and diagnostics.
- V300 non medical prescribing or willingness to work towards it.
- Problem solving, effective time management and prioritisation.
- Personal resilience and commitment.
- Ability to work under pressure, multi task, and travel independently across the Trust.
- Work flexibly in accordance with service needs.
- Relevant and current experience in urgent care and/or primary care with extensive clinical skills and knowledge.
- Experience in an emergency/first contact role and/or crisis management.
- Evidence of continuing professional development.
- Experience in a variety of settings, including hospital and community, with knowledge of chronic and long term conditions.
- Integrated team working across health and social care.
- Advanced clinical skills and specialist knowledge.
- Ability to work as an autonomous practitioner and demonstrate advanced clinical judgement.
- Effective communication, self management, and cultural competence.
- Knowledge of legal and ethical issues.
Bridgewater Community Healthcare NHS Foundation Trust and Warrington and Halton Teaching Hospitals NHS Foundation Trust plan to integrate and form a new organisation on 1st April 2026. If your start date is on or after 1st April 2026 you will be employed by the newly formed organisation.
