Applicants who do not meet the essential criteria will be considered under the Annex 21 training programme, where whilst undergoing training, they will be paid a 70% of the top of Band 7 and on completion of training will then move on to Band 7 scale. About The Organisation

The Role
  • The Advanced Practitioners will work autonomously across Primary, Community and Acute Services during the in and out of hours period, covering seven days including public holidays. They will be responsible for the clinical decision making for the population including assessment and treatment of patients in across the Primary, Community and Acute settings.
  • The Advanced Practitioner will work closely with the multidisciplinary team (MDT)/medical staff including the OOH Hub and Acute Services. Communicate on a daily basis about any patient they have seen overnight/ at weekend and who needs to be followed up by the GP / Acute Services during normal working hours
  • The Advanced Practitioner provides assessment, clinical care and management of both stable and acutely unwell patient and his/her family, making complex, autonomous decisions using advanced skills and knowledge, having overall responsibility for the assessment of care needs and the delivery of advanced care. Medical telehealth/telephone support will be available when required from an identified source.
In this key role, you will:
  • Autonomously assesses, diagnose and treats individuals with a range of acute, non acute and chronic presenting medical conditions across lifespan
  • As an Advanced Practitioner, support the AHP/Nursing team to ensure that the appropriate clinical skills and competencies are used effectively to meet the needs of both acutely unwell and stable patients and their families
  • Undertakes the management of a caseload of individuals, taking autonomous action to organise and undertake a wide variety of investigative procedures to support clinical practice, and optimise the patient experience and take responsibility for the patient journey
  • Develop and actively participate in research by working collaboratively with multidisciplinary and academic partners and other HEIs
What You'll Bring:
  • Registered Nurse / AHP / Pharmacist with valid NMC/HCPC / GPHC Registration.
  • Advanced Specialist knowledge and experience of relevant conditions, pathology, policies and procedures associated with the speciality/area of practice ensuring that the level of expertise can be utilised to deliver leadership within speciality
  • The post holder will possess excellent team-working/leadership skills and have the ability to motivate and influence others and work using own initiative
  • Comprehensive knowledge of clinical guidelines and standards within the speciality
  • A driving licence is required; this must be a full UK/EU/EEA licence.

From 1 April 2026, the Agenda for Change full-time working week will reduce from 37 hours to 36 hours. Part time working hours will be reduced on a pro ratio basis. A corresponding increase in the hourly rate will apply, ensuring that overall pay remains unchanged.
