Job Description
Job Summary 
We are looking for a highly motivated and compassionate Advanced Nurse/Clinical Practitioner (ACP/ANP) to join our established team of ANPs working 22.5 hours per week.
Responsibilities- Provide high standard patient care to patients of all ages using advanced autonomous clinical skills and comprehensive theoretical knowledge.
- Manage a clinical caseload and address presenting patient needs in a primary care setting.
- Contribute to the ongoing development of ANP/ACP services offered by the practice.
- Respond effectively to patient need, ensuring patient choice, shared decision-making and easy access to services.
- Make appropriate referrals to other clinicians or services.
- Mentor and support other clinicians in developing and maintaining clinical skills.
- Work effectively as part of the practice team to ensure high quality care for all patients.
- Make autonomous clinical decisions for which you are accountable.
- Act as the first point of contact for patients presenting with undifferentiated, undiagnosed problems, using history taking, physical examination and clinical decision making to establish diagnosis and management plan.
- Initiate necessary invasive and non invasive diagnostic tests, interventions and interpret findings.
- Prescribe safe, effective and appropriate medication as an independent prescriber within current legislative framework.
- Provide safe, evidence based, cost effective and individualized patient care.
- Offer a holistic service, developing ongoing plans of care/support with emphasis on prevention and self care.
- Promote health and well being through health promotion, health education, screening and therapeutic communication skills.
- Refer patients directly to other services/agencies as appropriate.
- Collaborate with nursing, medical and healthcare assistant colleagues to deliver National Service Frameworks (e.g. Coronary Heart Disease, Older People, Diabetes, Mental Health).
- Collaborate with the wider Clinical Commissioning Group to promote integrated and seamless pathways of care.
- Contribute to the practice achieving quality targets and sustaining high standards of patient care and service delivery.
- Participate in identifying community health needs and develop patient/family centred strategies to address them.
- Help develop and set up new patient services and participate in initiatives to improve existing services.
- Promote a learning environment for patients, nurses and other health professionals.
- Plan and implement teaching for existing staff, such as medical students, nurse practitioner students, GP registrars and health care assistants.
- Promote evidence based practice through the latest research based guidelines and practice based research.
- Monitor the effectiveness of your clinical practice through audit, peer review and quality assurance strategies.
- Maintain professional registration and participate in continuing professional development to keep up to date with evidence based knowledge and competence.
- Work within professional Codes of Conduct and keep up to date with relevant health related policy.
- Collaborate with colleagues within and external to the practice and demonstrate leadership.
- Proactively promote the role of the ANP/ACP within the practice, externally to key stakeholders and agencies.
- Maintain effective communication pathways with all members of the primary care team and other agencies.
- Travel independently between practice sites and attend meetings hosted by other agencies as required.
- Handle contact with body fluids (e.g., wound exudates, urine) safely during clinical practice.
- Adhere to safeguarding policies and participate in required safeguarding and statutory training.
- Undergo a Disclosure and Barring Service check as required by law.
- Accept that the position may open to additional duties or relinquish existing duties to maintain efficient practice operation.
- Independent Non Medical Prescriber.
- Current NMC or professional body registration.
- Demonstrated advanced clinical practice.
- Experience in prioritising needs and informed decision making in care and delivery.
- Strong organisational and team working skills.
- Excellent communication skills, able to relate at all levels.
- Excellent interpersonal skills, able to work independently and with others to deliver high class service.

- Masters qualification in Advanced Clinical Practice.
- Recognised teaching/assessment qualification.
- Specialist course relevant to primary care (CHD, Diabetes, etc).
- Experience developing services based on assessed patient group needs.
- Knowledge of current primary care organisational policy and national legislation.
- Managerial responsibilities, such as maintaining equipment, ordering supplies within financial resources, managing ANP cover and holiday rota, and conducting appraisals.
- Acute and emergency care experience (A&E).
- Able to demonstrate competencies across a range of disease areas.
- Able to demonstrate critical analytical skills and problem solving.
- Ability to initiate innovation in practice.
- Ability to work across professional and organisational boundaries.
- Presentation skills.
- Understanding of computer systems and Read code usage in general practice.
- Data analysis skills, including use of audit to review service and care provided.
