Exciting Opportunity: Advanced Nurse Practitioners - Community Medicine for the Elderly ANP Team (Home First Angus)

We are delighted to offer an exciting new opportunity to work in Angus within a brand-new, innovative service. Angus Health and Social Care Partnership is redesigning the way we deliver care, focusing on a model that brings together multi-agency teams to prevent unnecessary hospital admissions and support people to remain at home.

Our ambition is to ensure that individuals who do not require acute hospital admission-or who are already in hospital but no longer need acute care-are redirected and supported to receive the Right Care, at the Right Time, in the Right Place .

We are seeking experienced Advanced Nurse Practitioners (ANPs) who are ready for a new challenge and eager to contribute to the development of our Community MFE ANP Team. These are new posts within Angus HSCP and offer a unique opportunity to reshape community services through the Frailty at Home and Home First Angus - Single Point of Contact initiatives.

As an ANP, you will work closely with multidisciplinary teams, including Medicine for the Elderly Consultants, to:
  • Promote early discharge to home or a homely setting
  • Respond to new or existing health concerns in the community
  • Help reduce unplanned acute hospital admissions
  • Support advanced care delivery in Angus Community Hospitals
The role involves autonomous clinical practice at an advanced level, including physical assessment, diagnosis, planning and onward referral. In addition to providing daily clinical decision-making, leadership, and continuity of care, you will work closely with Enhanced Community Services (ECS), Social Work, General Practice teams, District Nurses, fellow ANPs and acute hospital colleagues to ensure smooth transitions between home and hospital. You will also contribute to day treatment assessments across our Community Hospital sites in Arbroath and Forfar.

Post Details:
These new posts are based at Whitehills Health & Community Care Centre, Forfar, as part of Home First Angus . Posts are 37 hours per week, worked over 7 days on a rota basis alongside the wider Home First and Single Point of Contact teams. Travel throughout Angus is essential.

Essential Requirements:
  • Registered on the UK NMC Register (Sub-part 1: RN1 or RNA or Sub-part 2: RN2)
  • Extensive post-registration clinical experience
  • Advanced clinical knowledge and professional practice, preferably in unscheduled or primary care settings
Desirable Requirements:
  • Postgraduate Diploma or Master's Degree in Advanced Practice
  • Independent Non-Medical Prescriber or working towards completion
Trainee ANP Roles:
We also welcome applications from those working towards a postgraduate diploma in advanced practice. Trainee ANPs will be supported through a combination of academic study, clinical supervision and mentorship. Applicants must demonstrate the ability to study at SCQF Level 11 and have completed relevant coursework aligned with the advanced practice pathway. Remuneration will follow Annex 21 paragraph 2(i), with consideration of current pay scale.
