Pioneers Integrated Partnership PCN are looking for a part time ANP to work at Tower Hill Partnership Medical Practice.
The hours available are 21 per week, ideally working 7 hours per day on Wednesdays, Thursdays and Fridays.
The role provides daily access to face to face appointments for patients that have been assessed via online triage as in need of being seen in person.
The role is available on a fixed term basis at present for 18 months, however, it is hoped to be able to extend the role dependent on future PCN funding.
Main duties of the jobThe Advanced Nurse Practitioner will be responsible for face to faceconsultations and attending to patients who wish to be seen by a Healthcare Professionalvia an online patient triage service through the Practice website. The role includes diagnosing and treatingpatients presenting with minor illness and seeing and advising people inrespect of their continuing medical and nursing needs.
The Advanced Nurse Practitioner will need to prioritise and triage theneeds of patients accordingly, making any necessary referrals forinvestigations in the appropriate manner and utilising PCN additional rolesstaff such as Social Prescribers, Care Coordinators and Pharmacists. Where appropriate, patients with problemsneeding referral to secondary care should be discussed with the registered GPbefore making such a referral.

Job responsibilitiesKeyroles and functions:
To work autonomously, providing a high quality,safe and supportive environment for patients, meeting their identifiedphysical and psychosocial needs within the scope of individualaccountable practice.
Work within Practice guidelines, policies,protocols and procedures reporting outcomes to appropriate clinical staffso standards of care can be monitored and improved where necessary. Thisincludes adhering to the following agendas: infection control, equalityand diversity, health and safety.

Protect and maintain patient records to theagreed Practice standard and in accordance with professional recordkeeping standards NMC 2009.
Administer medicines in accordance with anon-medical prescribing framework, patient specific direction or patientgroup direction in accordance with the standards for medicine management NMC 2007.
Organise, manage and prioritise own workloadensuring care of patients is paramount.
Develop the role by using evidence-basedpractice and continuously improving own knowledge as defined in the PREPhandbook NMC 2011 and revalidation guidelines.
Job responsibilities:
Diagnosing and Treating Patients Presenting with Minor Illness
Triageand treat patients presenting with minor illness and wishing to see ahealthcare professional; either in surgery or in their own home; making anynecessary referrals to other members of the primary healthcare team.
Advisepatients on general healthcare and minor ailments, with referral to othermembers of the primary or secondary healthcare team, as necessary.
Prescribemedicines in accordance with NMC Standards for Medicines Management 2007, NMCCode of Professional Standards 2015 and the Nurse Prescribing ImplementationPlan 1998.
Ensureclinical practice is safe and effective and remains within the boundaries ofcompetence and to acknowledge limitations.
Providing Assessment, Screening and Health Education:
MinorIllness / Minor Injury Clinics
Chronicdisease and admissions avoidance reviews and care planning
Healthscreening, contraception, dietary advice, blood pressure checks
Performor request blood tests, urinalysis, ECGs (if required
Providegeneral and specific health screening to the practice population, within agreedprotocols, with referral to other members of the primary and secondaryhealthcare team as necessary
Person Specification Experience- Minimum of 5 years post registration experience at a senior level (Band 7 or above) in specialty area
