Job Description

Barnet PCN5 are looking for two fixed term Advanced Nurse Practitioners to run clinics on Saturdays only, between 0900 and 1700.

Barnet Primary Care Network is made up of 6 well established GP Practices with a combined list size of approximately 50,000 patients.

Our clinical team consists ofGPs, Practice Nurses, HCA's, Clinical Pharmacists, First Contact Practitioners, Social Prescribing Link Workers, Mental Health Practitioners and Care Coordinators.

Main duties of the job

The post holder will undertake a variety of standard ANP duties including face-to-face consultations, some home visits and telephone consultations

Job responsibilities

Working independently and alongside GPs to identify appropriate care paths and treatment methods for patients through face to face appointments, telephone consultations and home visits

Making professional, autonomous decisions in relation to presenting problems whether self-referred or referred from other health care professionals

Using clinical nursing practice at an advanced level to deliver quality care and advice to patients and demonstrate an in depth knowledge of the options for achieving control of symptoms and assessing the health needs of patients with undifferentiated and undiagnosed problems

Perform safe triage of patients when necessary via telephone, video, e-consultation and face to face consultations

Prescribe medication appropriate to patient needs or refer the patient to a community pharmacy who will provide designated minor aliment services and prescription advice

Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions

Support and educate patients to adopt and promote healthy lifestyles and apply principles of self-care

Maintain a concept of nursing which incorporates physiological, social and spiritual components

Person Specification Qualifications
  • The successful applicant will preferably hold an MSc or Bsc Advanced Nurse Practitioner / Advanced Practice qualification, plus the Independent non-medical prescribing qualification
