Job Description
Overview

We are a well organised, supportive, innovative GMS practice operating from a purpose-built surgery in Kidderminster. We are looking for an Advanced Nurse Practitioner to join our friendly and supportive team. The ANP will be responsible for managing and providing care for the presenting patient with undifferentiated, undiagnosed conditions, from initial history taking, clinical assessment, diagnosis, treatment, prescribing and evaluation of care. To include acute, minor and long-term conditions in line with existing practice and national policies and guidelines. You will have experience of carrying out ANP duties in General Practice and will have a non-medical prescribing qualification. Salary: dependent on experience but BAND 8 equivalent. Main duties

The ANP will be part of a team of 3 ANPs and will be responsible for managing and providing care for the presenting patient with undifferentiated, undiagnosed conditions, from initial history taking, clinical assessment, diagnosis, treatment, prescribing and evaluation of care. To include acute, minor and long term conditions in line with existing practice and national policies and guidelines. 15 minute F2F appointments and a mix of telephone and face to face.

  • Assess patients with both minor and complex needs through history taking and physical examination, and use appropriate investigations to confirm diagnosis.
  • Examine and assess patient needs from physical, psychological and social perspectives and plan clinical care accordingly.
  • Plan, implement and evaluate an effective management plan, ensuring safe discharge/referral and transfer to appropriate health care services.
  • Diagnose and manage acute conditions, integrating drug- and non-drug-based treatment methods into the plan.
  • Perform safe triage of patients via telephone or face to face when necessary.
  • Prescribe medications appropriate to patient needs in line with evidence-based practice and practice protocols within scope of practice.
  • Provide information on prescribed or over-the-counter medications, including regimens, side-effects and interactions.
  • Prioritise health problems and intervene in complex, urgent or emergency situations, including initiation of emergency care when needed.
  • Support patients to adopt health promotion strategies and applicable self-care principles.

Aylmer Lodge is a large modern General Practice with 15,500 patients. We work from our main site in Hume Street Medical Centre and from our branch site Cookley Medical Centre.

Key duties and responsibilities

  • Management of Risk: assess and manage risk, ensure safety and infection control compliance.
  • Managing Information: use technology to plan and monitor care, and ensure accurate coding for audit.
  • Learning and Development: mentor junior staff, share learning, identify personal development needs, and educate patients and carers.
