Job Description
Advanced Nurse Practitioner - Marshall PCN

To work as an autonomous practitioner, working across up to fourGeneral Practices in the Romford area.

Responsible for the delivery of practice nursing services, workingas part of the PCN / practices multidisciplinary teams, delivering care withintheir scope of practice to the entitled patient population.

Main duties of the job

Responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, well women and well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.

This means we are able to build strong clinical teams within andacross the PCNs. We are now at the very start of an exciting journey ofbuilding a team of over 100 additional clinical roles to support and enable thedelivery of services within each of our 4 PCNs. We are building teams withinand across the PCNs to enable peer support and team working both within andacross professions.

Job responsibilities
  • Develop,implement, and embed health promotion and well-being programmes
  • Managepatients presenting with a range of acute and chronic medical conditions,providing subject matter expert advice to patients
  • Implementand evaluate individual specialised treatment plans for chronic diseasepatients
  • Identify,manage, and support patients as risk of developing long-term conditions,preventing adverse effects on the patients health
  • Provideadvanced, specialist nursing care to patients as required in accordance withclinical based evidence, NICE and the NSF
  • Providewound care (ulcer /Doppler etc.) to patients
  • Providespecialist clinics such as ear syringing, contraception etc.
  • Undertakethe collection of pathological specimens
  • Requestpathology services as necessary
  • Processand interpret pathology and other test results as required
  • Providechronic disease clinics, delivering INTERNET patient care as necessary, referringpatients to secondary / specialist care as required tui?
  • Maintainaccurate clinical records in conjunction with extant legislation
  • Ensureead codes are used effectively
  • Maintainchronic disease registers
  • Develop,implement, and embed well woman clinics
  • Develop,implement, and embed well man clinics
  • Develop,implement, and embed travel clinics
  • Develop,implement, and embed an effective call/recall system
  • Beresponsible for the implementation of an effective immunisation programme
  • Chaperonepatients where necessary
  • Prioritisehealth issues and intervene appropriately
  • Supportthe team in dealing with clinical emergencies
  • Recognise, assess, and refer patients presenting with mental health needs
  • Implementvaccination programmes for adults and children
  • Extendedand supplementary prescriber, adhering to extant guidance
  • Supportpatients in the use of their prescribed medicines or over the counter medicines(within own scope of practice), reviewing annually as required
  • Contributeto practice targets (QOF etc.), complying with local and regional guidance
  • Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
  • Delegateclinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
  • Supportthe clinical team with all safeguarding matters, in accordance with local and national policies
  • Understandpractice and local policies for substance abuse and addictive behaviour, referring patients appropriately
  • Deliver opportunistic health promotion where appropriate

Experience
  • Experience of working in primary care environment
  • Experience of chronic disease management
  • Experience of prescribing and undertaking medication reviews
  • Experience of working as a practice nurse or community nurse
