The Urgent Treatment Centre at Mid Cheshire Hospitals NHS Foundation Trust is looking for an Advanced Practitioner to join our small dynamic and friendly team, making a real difference in patient care. The team is a GP led service, made up of ACPs, senior nurses, technicians, and admin support staff.
The Advanced Care Practitioner will need to be competent in using their expert clinical knowledge and skills. They must have a full Masters and prescribing rights. They will have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis, and treatment of adults and children.
The role is offered either working short shifts (6 hours: 09 00 and 15 00) or longer shifts (11.5 hours: 09 00).
Main duties of the job- Require excellent communication skills - communicating clearly, sensitively, and effectively with patients, families, and professionals.
- High energy levels and a positive can do attitude to work alongside colleagues to deliver safe, kind, and effective care.
- Work autonomously, carrying out in depth clinical assessment of patients with complex multiple healthcare needs, diagnosing, treating, and discharging as appropriate.
- Work collaboratively, maintaining effective communication with other professionals and agencies to ensure patient needs are met.
- Demonstrate advanced communication skills and provide support when delivering diagnosis.
- Collect, collate, evaluate, and report information, maintaining accurate patient records.
- Participate in the development of services and assist the implementation of change.
- Provide education and development opportunities for others, supporting all members of the UCC team.
- Develop own specialist clinical knowledge and participate in the development of others' clinical knowledge.
Mid Cheshire Hospitals NHS Foundation Trust provides a full range of local hospital and community services for people across East Cheshire (population 399 K) and West Cheshire and Chester boroughs (population 357 K). Person Specification Essential- Registered General Nurse or AHP
- Masters Degree in Advanced Practice OR Evidence of Level 7 for Clinical Examination and Diagnostic
- Community Experience
- Excellent interpersonal and communication skills, both verbal and written
- Evidence of commitment to sustained professional development including leadership
- Evidence of clinical expertise and caseload management
