We are looking for an enthusiastic, experienced and forward thinking Advanced Clinical Practitioner to join our developing Hospital at Home service. This role focuses on frailty, urgent community care, admission avoidance and early supported discharge, working closely with the Community Geriatrician, Nurse Consultants and the wider multidisciplinary team to support patients in their own homes, care homes, and other residential care settings. The practitioner will be autonomous, with advanced clinical assessment, diagnostic reasoning and decision making skills, and a commitment to high quality, person centred care, service development and improving outcomes for patients in the community.

Key responsibilities- Provide advanced clinical practice across Hospital at Home, with a focus on Urgent Community Response and frailty pathways.
- Conduct advanced clinical assessments, formulate differential diagnoses, initiate investigations and develop evidence based management plans for adults with acute illness, frailty, respiratory conditions, functional decline, deconditioning and complex long term conditions.
- Support rapid response assessments, treatment planning, personalised care planning, advance care planning and escalation decisions across patients' own homes, care homes, residential care settings, Athorpe Lodge and Hospital at Home pathways.
- Collaborate with the Community Geriatrician and multidisciplinary team to support comprehensive geriatric assessment, rehabilitation goals, discharge planning and proactive management of patients with complex needs.
- Provide clinical leadership, advice and supervision to colleagues, contribute to admission avoidance, effective discharge, quality improvement, education, audit and service development.
- Degree level qualification.
- Teaching or mentorship qualification.
- MSc in Advanced Clinical Practice or equivalent.
- Evidence of ongoing professional development.
- Portfolio evidencing competency as an ACP.
- Qualified in Non Medical Prescribing (v3.0) if applicable for the ACP role.
- Current Advanced Life Support qualification (essential if inpatient or UECC ACP, desirable if community or non inpatient ACP).
- Current professional registration - NMC or HCPC.
- Demonstrated experience in a senior post.
- Experience working in multi disciplinary staff settings.
- Experience with acutely ill patients.
- Experience working independently.
- Experience in staff education and demonstration.
- Excellent communication skills with clear written and oral proficiency.
- Effective organisational abilities, supervisory skills, leadership and change management skills.
- Awareness of current health service priorities, national and local nursing and quality agendas.

Salary£57,528 to £64,750 a year, pro rata.
