Are you an experienced Advanced Clinical Practitioner, have you experience or an interest in General Practice and looking for a new challenge?
An exciting opportunity has arisen for an Advanced Clinical Practitioner to join our dedicated, dynamic, and friendly team at Marshalls Cross Medical Centre based within St Helens hospital. You may also work within the community including case managing and providing holistic medical care to patients within a busy care home.
You will be an integral part of Mersey & West Lancashire's clinical team supporting our GP surgery and care home residents. You will clinically support patients (both adults and children) with acute and urgent health conditions and with an emphasis on prevention of hospital care and or admission. You will further support our team with professional development, supervision and ensuring the most up to date evidence-based practice is implemented promoting the best patient experience and outcomes.
Previous experience within a Primary Care and/or Community setting is essential for this role.
Main duties of the jobThe post holder will work autonomously as a senior nurse within the Primary Care/Community environment, reviewing patients with undifferentiated and undiagnosed health problems; triage, taking comprehensive histories, undertaking clinical examinations, diagnosing, planning & implementing car, prescribing medication and making referrals where appropriate. You will also be responsible for supervising and supporting junior staff.

Person Specification Qualifications- Registered health professional with legal and accountable responsibilities for non-medical prescribing
- First level registered nurse/professional UK registration
- First Degree (or equivalent experience)
- Post registration course in specialty
- Immediate Life Support
- Masters in Advanced Clinical Practice
- Teaching qualification (C&G730, CertEd, PGCE or equivalent)
- Significant demonstrable experience of working in an acute care setting some of this must include working at a senior level managing and leading teams and this will be assessed at interview
- Ability to demonstrate specialist expertise underpinned by theory through CPD
- Excellent clinical reasoning /clinical decision making skills
- Evidence of involvement in the development of programs of care, protocols and guidelines
- Knowledge of Clinical risk management and Governance
- Excellent written and verbal communication skills demonstrating negotiation and persuasion skills
- Evidence of both theoretical and practical skills and competency in Care Management within the specialty
- Ability to manage demanding and challenging workload
- Evidence of both theoretical and practical skills and competency in IV therapy and care
- Ability to handle sensitive situations with tact and diplomacy
- Ability to make effective decisions
- Interpretation of diagnostic tests
- Ability to build rapport with colleagues, patients and relatives
- Experience of MDT working
