We are excited to welcome an experienced Advanced Clinical Practitioner (ACP) to join our compassionate and forward thinking Neighbourhood Team in Droitwich, Ombersley & The Rurals, delivering high quality clinical care to patients in their own homes.
This full time role offers flexibility in working hours and the chance to make a meaningful impact on the lives of our patients.
The role has a strong focus on frailty, complex care management, proactive admission avoidance, working collaboratively across the Primary Network Team (PCN) and wider multidisciplinary teams. You will provide comprehensive geriatric assessments (CGAs), diagnosis and treatment while contributing to service development and quality improvement across the four pillars of advanced practice. The post holder may also be required to provide clinical support within the Care Home Team, when service needs require to ensure continuity of care for residents. The role is ideal for someone who thrives in a collaborative environment and is passionate about improving patient outcomes and preventing hospital admissions.
This post will be at the start of the exciting new journey of the NHS 10 year health plan focusing on shifting care from hospital to local communities, prioritising neighbourhood based teams to manage long term conditions, frailty and complex needs, including the establishment of our Frailty MDT.
Main duties of the job- Provide advanced clinical assessment, diagnosis, and treatment planning for patients in their own homes, with a focus on frailty and complex needs.
- Lead on clinical decision making and contribute to complex case management.
- Actively participate in MDTs supporting integrated care across PCN.
- Collaborate with GPs, pharmacies, and care home staff to ensure continuity of care.
- Supporting ward rounds.
- Support medication reviews and optimise prescribing practices.
- Promote preventative care and early intervention strategies.
- Contribute to service development and quality improvement initiatives.
- Mentor staff and support professional development within the team.
- Apply the four pillars of advanced practice: Clinical Practice Leadership, Education, and Research.

Person Specification Experience- Demonstrable post graduate experience in a relevant area
- Extensive post graduate experience in a role undertaking assessments of complex needs
- Experience of working with people with long term illness and / or older people
- Clinical experience of caring for people with cognitive impairment
- Experience of working across professional boundaries
- Experience of change/leading service development
- Experience of clinical leadership
- Proven experience in a Band 8a ACP role or equivalent.
- Strong clinical leadership and decision making skills.
- Excellent communication and interpersonal abilities.
- Registered Nurse Level 1 / Allied Health Professional / Paramedic / Pharmacist.
- MSc Advanced Clinical Practice
- Non Medical Prescribing (V300) / ability to work within PGDs, within scope of professional practice.
- Advanced Assessment Skills Module
- Evidence of a portfolio of knowledge, skills and professional development
