Job Description

We are seeking a motivated and experienced Advanced Clinical Practitioner (ACP) to join our forward-thinking Primary Care Network. This is an excellent opportunity for a clinician who is confident in independently managing a broad range of acute and chronic conditions while delivering high-quality, patient-centred care within a supportive team and environment.

Main duties of the job

The successful candidate will work both autonomously and collaboratively within a multidisciplinary team, applying advanced clinical expertise and critical thinking skills to assess, diagnose, treat, and manage patients across the full spectrum of primary care presentations. They will demonstrate sound clinical judgement, professional accountability, and the ability to make safe and effective decisions within their scope of practice.

About us

Acclaim Primary Care Network (PCN) is a GP led organisation established in 2023. It has four member practices, with a combined list size in excess of 40,000 patients. The PCN provide enhanced services to patients across all four GP Practices, for example, weekly Care Home ward rounds, same day access clinics and Home Visits for acutely ill patients.

Job responsibilities

Key Areas of Responsibility

To work alongside the multi-disciplinary team across the PCN to assess and triage patients, including same day triage.

Provide definitive treatment such as the prescription of medications in accordance with guidelines (locally and nationally) and make referrals to other members of the primary care team.

Manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology and take appropriate action.

Undertake complex decision making to inform the diagnoses, investigation, and complete the management of episodes of care within a broad scope of practice. Proactively take a personalised care approach and population centred care approach to enable shared decision making with the presenting patient.

Person Specification Experience
  • Experience of working within multi-professional team environments.
  • Familiar with the interpretation of diagnostics and principles of shared clinical decision-making.
  • Leading multi-professional working within and across organisational boundaries.
  • Experience of working in Primary Care.
  • Have knowledge of chronic disease management reviews and QOF.
  • Experience of SystmOne.

Qualifications
  • Relevant professional qualification with affiliated professional registration
  • Masters level degree in Advanced Clinical Practice.
  • Evidence of Advanced Clinical Practice skills.
  • Evidence of continuing professional development.
