Job Description
Local Care Direct is a non-profit Social Enterpriseoperating across 24 hours a day, 7 days a week, 365 days a year, to providevarious primary and urgent health care services across West Yorkshire.
We are looking for an Advanced Clinical Practitioner
Salary:£29.02 per hour - £37.73 out of hours
Shifts Available: To be discussed at interview
Main duties of the job The AdvancedClinical Practitioner will practice, without direct supervision, as anindependent primary / urgent care healthcare practitioner within variousclinical environments, participating in care of all age ranges and presentingconditions.
The AdvancedClinical Practitioner will undertake assessment, diagnosis, treatment/referraland discharge of patients with primary/urgent care needs within their clinicalcompetence.
The AdvancedClinical Practitioner will provide clinical mentorship and leadership to alltrainee and peer practitioners within the workplace when required.

Job responsibilities
To ensure agreed lines of communication and reporting are implemented within the team to support all employees understanding LCD business plans, objectives, priorities and changes.
To promote effective cross-functional working and that the operational team supports the achievement of objectives, standards and protocols of other LCD areas.
To attend internal and external meetings representing LCD and engaging in discussions appropriate to the role and level of the post.
PATIENT CARE & SAFETY
To ensure that the quality of patient care and safety is firmly at the centre of the organisations objectives and vision.
To ensure the management and monitoring of the highest standards of safety and quality in the delivery of healthcare and related services, and manage risk within Local Care Direct.
Recognise and provide appropriate access and intervention so that all patients can receive person centre support, in line with organisational objectives to address/reduce Health Inequalities and promote Health Literacy.
FINANCIAL RESOURCES
To ensure financial policies and procedures are maintained and the budget is adhered to and that best value for money is achieved.
CONFIDENTIALITY
To operate and ensure the team operates within information governance protocols and that business confidentiality is maintained at all times.
TRAINING
To complete the defined company induction programme and core training for this role and mandatory refresher training thereafter and ensure that accurate records of training are maintained.
INFECTION PREVENTION AND CONTROL
Healthcare workers have a duty of care to patients and are expected to comply fully with the companies Infection Prevention and Control (IP&C) policies and attend/complete IP&C training, for personal and patient safety and for prevention of healthcare-associated infection (HCAI). Job responsibilities
- Independently prioritise, assess, treat, and safely discharge/refer patients attending with undifferentiated and undiagnosed urgent primary health care problems
- Undertake a range of clinical consultations via a range of modalities primarily face to face but may include telephone and home visits (service specific).
- Act as a referral agent and care co-ordinator by establishing multi-agency collaboration across the primary/secondary interface to meet the needs of the patient.
- Work within the parameters of current prescribing legislation or approved Patient Group Directives.
- Provide professional and clinical advice and direction to colleagues within your team.
- Maintain accurate, contemporaneous records.
- Demonstrate evidence-based care, working within agreed guidelines and protocols.
- Participate in audit, data collection and activity monitoring to improve performance and inform future service developments.

Person Specification Qualifications
- One of the following:
- NMC Registered General Nurse
- OR
- BSc in Paramedic Science/IHCD Paramedic qualification
- OR
- BSc in Physiotherapy
- OR
- MPharm
- AND Advanced Clinical Practitioner MSc/Post Grad Diploma in advanced clinical practice - minimum 1 year post Masters study.
- Current registration with HCPC, NMC or GPhC.
- Spotting the sick child.
- Level 7 Minor Injury Qualification.
- MSc or PGdip in advanced practice - or equivalent - mapped to all outcomes on the Multiprofessional advanced practice framework HEE.
- HEE digital badge.
- NHS credentialing via the supported e portfolio route.
- Effective communication skills: verbal, written and non-verbal.
- Professionally accountable for clinical practice.
- Knowledge of current clinical and professional issues.
- Up-to-date knowledge of health agenda and health inequalities (local and national).
- Innovative approach to practice.
