Job Description
Advanced Clinical Practitioner

The closing date for applications is 20 January 2026.

The post holder will be an experienced professional who, acting within their professional boundaries, will provide comprehensive care for patients from presentation through to recovery. This includes history taking, clinical assessment, diagnosis, treatment and evaluation of care.

They will demonstrate safe, advanced clinical decision making and expert care, including assessment and diagnostic skills for patients within the community.

The post holder will demonstrate critical thinking in the clinical decision making process. They will work collaboratively with GP's, community nursing teams, community geriatrician and the wider Multi Disciplinary Team (MDT) to meet the needs of the patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. NMC requirements for advanced practice must be met.

Work within the infection control agenda to improve cleanliness, improve patient environments, and resolve problems for patients and their carers by building closer relationships.

Main duties of the job
  • Assess, diagnose, instigate appropriate investigations, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
  • Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs.
  • Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long term condition (as appropriate).
  • Diagnose and manage both acute and chronic conditions, integrating drug and non drug based treatment methods into a management plan.
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs in accordance with evidence based practice and national and practice protocols, and within scope of practice.
  • Work with patients to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over the counter medication regimes, side effects and interactions.
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self care.
  • Demonstrate effective and compassionate communication and leadership skills to optimise your impact.

Person Specification Experience
  • Significant experience of working as a registered practitioner in a provider organisation (e.g. acute sector, community provider, third sector, social care or primary care) and within a relevant specialty.
  • Evidence of recent teaching, mentorship and supervision of staff.
  • Evidence of recent audit and/or research experience.
  • Experience of multi professional working.
  • Evidence of involvement in change management and/or clinical service improvement.
Knowledge
  • Up to date and detailed knowledge of relevant guidelines and clinical practice in the speciality.
Skills / Abilities
  • Evidence of theoretical and practical skills allowing autonomous patient management (i.e. patient assessment, diagnosis and management) related to clinical scope of practice.
  • Understanding, encouraging and supporting clinical supervision and mentorship both within and across professional roles.
  • Effective leadership skills relevant to the role (i.e. demonstrated ability to provide clinical professional leadership).
  • Effective written and verbal communication skills.
  • Educational skills such as identification of learning needs, delivery and evaluation, providing work based learning.
Communication Skills
  • Able to demonstrate a good standard of English language.
  • Ability to communicate effectively with all stakeholders including service users, clinicians and senior leadership teams. Proven leadership skills demonstrating motivation, vision, diplomacy and negotiation.
Planning and Organising Skills
  • Ability to interpret and evaluate national and international guidelines and apply to local protocols and practice as appropriate.
  • High level problem solving and analytical skills and manage complexity.
  • Effective time management and prioritisation skills.
Physical Skills
  • Works under own initiative with strong organisational skills and ability to meet deadlines.
  • Appropriate level of IT literacy (i.e., radiology, pathology, microbiology ordering as per Trust protocols, utilising patient systems).
Other
  • Willingness to use technology to improve standards of care and support to patients.
  • Access to a vehicle for travelling within and outside the trust boundaries.
Qualifications
  • Registered 1st level nurse or equivalent.
  • Clinical supervision training.
  • Advanced Clinical Practitioner qualification MSc.
  • Long term conditions qualification.
  • Intermediate Care Experience.
