Advanced Clinical Practitioner - General Practice Visiting Service

We have an amazing opportunityfor a highly motivated and dedicated ACP to join ourestablished multidisciplinary team of clinicians, supporting our innovative carehome, acute and home visiting service, whilst we in turn support your personaldevelopment with annual appraisal, training and education.

Work as an effective andresponsible team member supporting others and exploring the mechanisms todevelop new ways of working to benefit the practices and the patients.

Working within the EnhancedHealth in Care Homes (EHCH) model we are supporting people living in care homeswho can expect to receive the same level of support as if they were living intheir own home.

Supporting our houseboundpatients to improve the management of LTCs and help reduce admissions.

Triaging and attending acutehome visits in rotation with the clinical team.

Our aim is working together toprovide high quality, accessible, patient focused and sustainable healthcare.

Main duties of the job

Working with our existing cliniciansto support the work of the GPs in the practice with care-home visits andward-rounds, housebound patients and acute home visiting.

Weekly care home rounds, liaisingclosely with care home staff, practice GPs and colleagues in support of theirresidents.

Support the GPs by triaging and attendingacute home visits in rotation with the established clinical team. To manage acute illnesses in patients who are notable to attend the surgery, may include arranging admissions to hospital orliaising with community teams, palliative care teams and social care.

Deliveringsupport to our housebound patients. To improve management of LTCs, help withdecisions around ceilings of treatment and reduce hospital admissions.

To delivera high standard of patient care, using advanced autonomous clinical skills, anda broad in-depth theoretical knowledge base.

To contribute to the QOF/IIFtargets for the Practices and support the ongoing development of both the carehome and visiting service.

Wyvern Health is a Primary CareNetwork (PCN) of three GP practices, Priory Road Medical Centre, Lawn MedicalCentre and Old Town Surgery.

We are high performing practiceswith a strong ethos of continuity of patient care, teamwork and peer support.We have a list size of circa 27,500 patients, our clinical system is SystmOneand our most recent CQC rating was GOOD.

The PCN has 10 GP partners and an extensive clinicalteam of advanced clinical practitioners, general nurses, pharmacists, pharmacytechnicians, first contact physiotherapists, health care assistants, care co-ordinators and admin support staff,working alongside a management team and sharing best practice to benefit both our teams & ourpatients alike.

Job responsibilities

Clinical practice:

To include ward rounds atnursing/care homes to deal with clinical queries some which may be urgent toavoid admission, medication reviews, referrals and end of life care.

Manage requests from patientsrequiring acute home visits. Triaging appointments as well as visiting.

Support in the care of housebound patients.

Ensure the highest standards oftreatment and care for all patients (including complex patients, both acute andchronic). This will involve diagnosis, implementation of care plans to ensurephysiological and psychological factors are taken into consideration andongoing evaluation of any plans to ensure patient compliance.

Working closely with our team ofPharmacists to prescribe and review medication for therapeutic effectiveness,appropriate to patient needs and in accordance with evidence-based practice andnational and practice protocols, and within scope of practice.

Health promotion and motivationof patients to improve general health.

Communicate effectively withpatients and carers/care home staff (nurses/carers), recognising the need foralternative methods of communication to overcome different levels ofunderstanding, cultural background, and preferred ways of communicating.

Recognising issues and gatheringsufficient information to refer (e.g. drugs; domestic violence; childprotection; vulnerable adults; senior patients; social problems).

Manage and assess risk withinthe areas of responsibility, ensuring adequate measures are in place to protectstaff and patients.

Be mindful of work areas andpractices to ensure they are safe and free from hazards and conform to health,safety and security legislation, policies, procedures and guidelines.

Follow all necessaryinfection-control measures according to local and national guidelines.

Person Specification Experience
  • 5 years post registration experience.
  • Recent experience within primary, community or emergency setting.
  • Ability to work under pressure.
