Advanced Clinical Practitioner - Crisis Response / Hospital at Home

Are you an ACP who wants to work within a community based MDT and support patients to stay at home, reducing the need for hospital admission? We're looking for an enthusiastic Advanced Clinical Practitioner to join our Community Response Team and support patients to stay at home, reducing the need for hospital admission.

As an established Crisis Response service, you will work with an MDT alongside other ACPs to provide vital care, completing complex assessments and minimising hospital admissions. The new Hospital at Home service enables patients to receive safe, effective treatment at home for up to 14 days, with referrals from community services, NWAS, 111, GPs and ED deflections.

In this role you will work on a rotational basis between Hospital at Home and the Urgent Care Crisis Response Team, develop key links with acute and community colleagues, support the multi disciplinary team, deliver education and participate in service improvement initiatives.

Main duties of the job
  • Work across hospital and community to develop key links and networks with acute and community colleagues.
  • Use specialist clinical skills to determine the most appropriate pathways for patients and provide virtual patient monitoring where needed.
  • Work closely with the multi disciplinary team across acute, community and voluntary partners to ensure safe and effective discharges and prevent re admission.
  • Support other team members, providing education and support to deliver a high quality service for patients.
  • Participate in both internal team projects and wider trust initiatives, forming professional relationships across a wide variety of multi disciplinary team members.

Qualifications
  • MSC in Advanced Practice
  • Relevant professional health degree or equivalent experience
  • Evidence of recent CPD
  • Independent non medical prescriber (V300)
  • Current registration with NMC, HPC and/or other statutory body
  • Communication/counselling skills training
Experience
  • Substantial post registration experience, including leadership experience at Band 7 or above
  • Proven experience of working as an independent practitioner
  • In-depth knowledge of recent NHS legislation relevant to community care
  • Experience of leading, planning and completing audit/research
  • Experience of teaching and mentoring staff across the multi disciplinary team
  • Evidence of applying research in practice
  • Leadership and qualification experience
Skills and knowledge
  • Advanced clinical skills and autonomy as a practitioner
  • Evidence based practice and acute disease management experience
  • Leadership skills with ability to engage and deliver change through negotiation and partnership working
