Advanced Clinical Practitioner (ACP) - AVS Team Lead role at Bury GP Federation. Lead the Acute Visiting Service (AVS) team across the Bury Primary Care Networks (PCNs). Combine advanced clinical practice, leadership, education, and service development to deliver high-quality, responsive care to patients in their homes and local communities. Manage and mentor a multidisciplinary team, support professional development, and drive continuous improvement in the service.
Main responsibilities- Provide autonomous, high-quality clinical care including same-day and scheduled consultations, home visits, and specialist clinics.
- Lead, support, and manage the AVS team, undertaking appraisals and supporting ongoing professional development.
- Represent Bury GP Federation at locality and regional meetings, promoting collaboration across the health and care system.
- Deliver clinical supervision and act as a prescribing or clinical supervisor for trainee ACPs and other clinicians.
- Lead on service development, governance, and quality improvement projects.
- Support research, audit, and evaluation activities within the service.
- Promote equality, diversity, and inclusion in all aspects of practice.

- Hours: 37.5 hours per week
- Contract: Permanent
- Working pattern: Full-time (Monday-Friday, 10:00-18:00, with occasional out-of-hours meetings)

Person specification Qualifications- Registered healthcare professional (NMC / HCPC / GMC) in good standing.
- MSc in Advanced Clinical Practice.
- Experience working autonomously in primary or urgent care.
- Demonstrated leadership or management experience.
- Excellent verbal and written communication skills.
- Strong understanding of primary care delivery within NHS England.
- Mentoring or coaching qualification.
- Experience supervising postgraduate learners or trainee ACPs.
- Knowledge of mental health, MSK, cancer, and chronic disease management.
- Experience in service improvement or contractual performance management.
