Thisis a great opportunity for a registered Paramedic/Nurse practitioner or atrainee advanced care practitioner who is currently undertaking Advanced CarePractitioner training.
Main duties of the jobYou will work in a friendly, multi-disciplinaryteam, working alongside GPs assessing and triaging patients in the minorillness clinics and conducting home visits. This will include same day triageand as appropriate provide a definite treatment or make necessary referrals toother members of the primary care team.
Youwill also:
- Act as a key contactfor Care Homes and patients on the frailty register, offering proactive care.
- Advise patients ongeneral healthcare, including self-management and signposting,
- Undertake thecollection of pathological specimens,
- Support the team withclinical emergencies,
- Liaise with externalservices/agencies to ensure patients are supported appropriately (vulnerablepatients etc)
Flansham Park Health Centre is atraining Practice with 4 dedicated GP Partners and 8 Salaried GP's supported bya team of highly professional clinical and non-clinical staff to provide thehighest possible standard of care to over 13,200 patients.

Job responsibilitiesThefollowing are the core responsibilities of the Advanced Care Practitioner. Theremay be, on occasion, a requirement to carry out other tasks; this will bedependent upon factors such as workload and staffing levels:
- Work as part of a multi-disciplinary team (MDT)
- Assess and triage patients, including same day triage, and asappropriate provide a definite treatment (including prescribing medicationsfollowing policy, patient group directives, NICE (national) and local clinicalguidelines and local care pathways) or make necessary referrals to othermembers of the primary care team.
- Advise patients on general healthcare and promote self-managementwhere appropriate, including signposting patients to the organisations socialprescribing service and where appropriate, other community or voluntaryservices
- Provide advanced, care topatients as required in accordance with clinical based evidence, NICE and Undertakethe collection of pathological specimens
- Request pathology/investigatory procedures
- Process and interpret pathology and other test results as required
- Maintain accurate clinical records in conjunction with extantlegislation
- Ensure read/SNOMED CT codes are used effectively
- Prioritise health issues and intervene appropriately
- Support the team in dealing with clinical emergencies
- Be an extended and supplementary prescriber, adhering to extantguidance, working towards or willing to undertake this training
- Support patients in the use of their prescribed medicines or overthe counter medicines (within own scope of practice), reviewing annually asrequired
- Contribute to practice targets (QOF etc.), complying with localand regional guidance
- Liaise with external services/agencies to ensure the patient issupported appropriately (vulnerable patients etc.)
- Delegate clinical responsibilities appropriately (ensuring safepractice and the task is within the scope of practice of the individual
- Support the clinical team with all safeguarding matters inaccordance with local and national policies
- Understand practice and local policies for substance abuse andaddictive behaviour, referring patients appropriately
- Performspecialist health checks and reviews within their scope of practice and in linewith local and national guidance
- Performand interpret ECGs
- Supportthe delivery of anticipatory care plans and lead certain services (e.g.,monitoring blood pressure and diabetes risk of elderly patients)
- Communicateat all levels across organisations ensuring that an effective, person-centeredservice is delivered
- Communicateproactively and effectively with all colleagues across the MDT, attending andcontributing to meetings as required
- Communicateeffectively with patients and, where appropriate, family members and theircarers, where applicable, complex and sensitive information regarding theirphysical health needs, results, findings and treatmentchoices
- Currently training as an Advanced Care Practitioner
- Educated to degree level in paramedicine/Nursing or equivalent experience
- Registered with the Health and Care Professionals Council as a paramedic or registered with the NMC
- Post graduate qualifications in minor injury/illness and advanced patient assessment
- Experienced in dealing with a range of clinical conditions
- Completed two-year Consolidation of Learning period when a newly qualified paramedic
- Experienced in triage
- Experience of working in a primary care environment
- Ability to listen, empathise with people and provide person centered support in a non-judgmental way
