Adult's SW - Community Team - Herefordshire

Your key responsibilities will include:

• Conducting assessments to identify the needs and preferences of individuals.
• Developing and implementing personalized care plans in partnership with service users.
• Providing ongoing support and intervention, ensuring safeguarding and promoting independence.
• Working closely with multidisciplinary teams and external agencies to coordinate care and resources.
• Maintaining accurate and up to date case records in compliance with legislation and best practices.

The ideal candidate will possess the following essential qualifications and skills:

• A recognized social work qualification (e.g., BA/MA in Social Work, CQSW, DipSW).
• Registration with Social Work England.
• Demonstrable experience in adult social work, particularly in community settings.
• Strong assessment, communication, and interpersonal skills.
• Ability to work independently and as part of a team, managing a diverse caseload.
