About the Liverpool Adults and NHS Outreach Team

Our support workers, operating within the adult and NHS services, provide personalized 1:1 support to all our service users. Our dedicated support workers conduct meaningful independence sessions with each service user, focusing on preparing them for independent living. These sessions cover essential aspects such as financial management, property maintenance, and guidance on employment seeking, tailored to the unique needs of each individual.

You will be providing both practical and housing related support to adults or families in temporary accommodation, across the Liverpool region. It involves multi agency work and would suit anyone who has a desire to help adults or families achieve important life and housing related skills to take forward to their permanent homes.

Due to the areas this role covers a car driver with a vehicle and business insurance is essential.

The ideal candidate will be passionate about this sector, looking to make a positive difference to the lives of our service users and will be looking for a challenging but rewarding role.

Qualifications
  • Previous experience supporting disadvantaged people, families or young people to gain independence (non care work).
  • Experience with complex caseloads including mental health and challenging client groups.
  • Must hold a full driving licence and have access to a vehicle insured for business use.
  • Must be prepared to travel throughout Liverpool.
  • Thorough knowledge of safeguarding adults and children.
  • Have or be willing to work towards an NVQ Level 3 Health and Social Care qualification.
  • Good interpersonal skills.
Key Duties
  • Managing a caseload of service users, helping them build independent living skills.
  • Liaising with relevant professionals such as social workers, health care practitioners and the police.
  • Writing detailed accurate reports assessing a service user's independent living skills.
  • Completing weekly inspections of service user properties; a background in housing would be desirable.
  • Supporting access and maintenance of education and employment.
  • Promoting physical, mental and emotional well being, de escalating challenging situations when they arise.
  • Signposting to appropriate services as required, including mental health, drug rehabilitation, counselling and education.

Salary

£34,375.24 per annum inclusive of sleep rate.

Up to £32,017.38 inclusive of sleep rate.
