Adult Social Care Practitioner - Early Support Team

We Do Social Work in Manchester is recruiting an experienced Adult Social Care Practitioner to join the Adult Early Support Team (AEST). The role focuses on strengths-based, preventative support from first contact to help adults remain independent.

You will work in a fast-paced, multidisciplinary environment, assess needs, build partnerships with families and professionals, and develop person-centred solutions to promote wellbeing and prevent escalation of care.
