Team Lead

Overview

We are seeking an enthusiastic and highly-motivated Team Lead to join our Dementia Outreach Team and become part of a service that really makes a difference to patients experiencing a dementia-related crisis, and their families.

Responsibilities

• Provide clinical and managerial leadership with mentoring and educational input to the Dementia Outreach Team multidisciplinary team.
• Act as a clinical leader and expert practitioner liaising, guiding and advising the multidisciplinary team in the provision of optimum patient experience for people accessing support and assessment.
• Support the effective role modelling, ongoing management and clinical supervision, and mentorship to the team.
• Ensure that the highest quality care is carried out in accordance with policies and procedures Hampshire and Isle of Wight NHS Foundation Trust.

Qualifications

• You will ideally have experience of working with people with dementia and one of the professional registrations outlined within the attached Job Description and Person Specification.
• The ability to work in partnership with carers/family members and services in the community is essential.
