Care Home In Reach Nurse

30 HOURS PER WEEK

We have an exciting opportunity for a Qualified RMN or RGN to join our well established care home in reach team, a countywide service.

The team supports residents of care homes living with dementia or those over 65 with a functional mental health illness.

The service operates Monday to Friday, 9-5, excluding bank holidays. Flexible working patterns will be considered. The team is based at the grounds of Belmont Abbey, a peaceful rural location 2 miles from Hereford, with plenty of free parking.

Responsibilities
  • Manage a caseload of patients to ensure timely assessment, diagnosis and treatment.
  • Monitor treatments and implement changes to improve effectiveness.
  • Perform effective and timely discharge planning and coordination.
  • Undertake and understand physical health observations, taking appropriate action when necessary.
  • Monitor, evaluate and modify treatments to measure progress.
  • Apply high-level understanding of dementia and mental health conditions, providing advice on management, healthy lifestyles and wellbeing.
  • Carry out complex risk assessment.
  • Provide timely and accurate reporting and escalation of incidents and safeguarding concerns.
  • Attend weekly multidisciplinary meetings.
  • Act consistently with the values and beliefs of the organisation and promote these on a day to day basis.
  • Serve as a role model to colleagues, maintaining the highest standards of professionalism.
  • Use initiative and take responsibility for own work and service quality.
  • Work as part of the multidisciplinary team across clinical pathways.
  • Manage complex needs using evidence based, patient centred principles.
  • Support patients and carers to achieve outcomes via the recovery model.
  • Provide leadership for junior staff through supervision and appraisal.
  • Participate in planning, development and evaluation of services and manage defined projects.
  • Supervise students on practice placement.

Experience
  • Documented evidence of continuing professional development.
  • Significant post registration experience in mental health.
  • Minimum 1 year within related specialty.
  • Extensive clinical experience, including individual and group work.
  • Experience in varied mental health settings.
  • Supervision of junior staff and students.
  • Leadership and first line management experience.
  • Implementation of psychological therapies.
Knowledge
  • Specialist knowledge and application of relevant interventions.
  • Knowledge and experience of audit procedures.
  • Detailed knowledge of mental health legislation and current practice.
  • Knowledge of clinical governance principles and application.
  • Knowledge of Person Centred Care.
  • Knowledge of risk assessment.
  • Multidisciplinary team working.
  • Knowledge and application of standardised assessments.
Qualifications
  • RMN (Degree/Diploma and working towards degree).
  • Evidence of continuing professional development.
  • Dementia training.
