Join an innovative, fast evolving integrated healthcare service. This exciting opportunity is for a motivated, patient centred Senior Pharmacist to join Worcestershire Hospital at Home team. Working across a place based model, you will use advanced clinical skills within a multidisciplinary team to support patients receiving hospital level care at home. Collaborating with senior clinicians across acute, community and neighbourhood services, you will help reduce admissions and readmissions, support safe discharge and transfers, and improve medicines optimisation and safety.
This role offers autonomy and the chance to shape the future of Hospital at Home services, contributing to service development, quality improvement, and pathway redesign. This is a full time post.
Main duties of the jobYou will need to be a confident decision maker, with clear leadership skills, excellent pharmaceutical knowledge and technical skills and a sound understanding of pharmacy service and hospital service.
Lead, deliver, develop, and assess the value of clinical pharmacy services to support Hospital at Home service.
Support the development of good practice and safe medicines management and optimisation systems across Herefordshire and Worcestershire Integrated Care System (ICS).
Work in collaboration with different organisations across the IC to provide highly effective clinical pharmacy services to patients within Hospital at Home service.
Develop and work within appropriate governance structures for the designated clinical service liaising with IC stakeholders as needed.
Provide clinical pharmacy teaching to relevant staff across all grades and disciplines.

- Significant knowledge of the national pharmacy agenda
- Management of resources and delivery of projects
- GPhC registered Pharmacist
- Master's degree in pharmacy or pre 2000 equivalent
- Diploma in Clinical Pharmacy or equivalent demonstrable experience
- Evidence of extensive post registration study/CPD
- I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
