The Care Partner (Endorsed Enrolled Nurse) is a dual role position responsible for the coordination and delivery of high quality, person centred care for older people living in the community. This includes providing direct nursing care and support within the scope of an Enrolled Nurse, as well as partnering with clients and their families to plan, monitor, and review home care services.

The Care Partner (Enrolled Nurse) will report to the Director of Home Care. The role is delivered under the wellness and reablement model and in alignment with the Strengthened Aged Care Quality Standards (2025), ensuring clients' physical, emotional, and social well being are optimised.

About the role

Perform the duties of the role in accordance with CatholicCare's Mission, Vision and Guiding Principles

As a key member of our team, you'll coordinate, monitor, and support the clinical care needs of clients receiving home care services. You'll also manage a caseload of Home Care Package clients and work closely with Clinical Care Partners to assess and address clinical needs.

Responsibilities
  • Provide evidence based nursing care and health monitoring to clients in the community within scope of practice.
  • Act as the key contact for a portfolio of clients, coordinating service delivery and supports in alignment with goals, assessed needs and preferences.
  • Build strong, respectful partnerships with clients, families, and carers.
  • Ensure accurate and timely documentation of all clinical and service related activities.
  • Engage in regular supervision, education, and competency training.
  • Comply with all WHS policies and procedures.
About you
  • Ability to undertake the duties of the role in accordance with CatholicCare's Mission, Vision and Values
  • Diploma of Nursing and current AHPRA registration as an Enrolled Nurse (with medication endorsement)
  • 2 years Endorsed Enrolled Nurse experience
  • Demonstrated experience in aged care, community nursing, or care coordination
  • Knowledge of Aged Care Quality Standards and person centred care principles
  • Strong assessment, care planning, and clinical documentation skills
  • Excellent interpersonal, communication, and relationship management skills
  • Proven ability to manage a caseload and prioritise competing demands
  • Excellence in communication skills both verbal and written
  • Competence in computer skills and demonstrated experience and skills working with database systems
  • Highly organised with ability to plan and manage a varied workload in a fast paced environment
  • Ability to work as part of a team, engage with people from various community settings, socioeconomic and cultural backgrounds to meet required organisational and client outcomes
  • Ability to change own model of practice to align with the reformed aged care sector, including Clinical wellness with a focus on having clients identify and work towards achieving outcomes
  • Experience responding to issues identified by clients in a resource efficient and timely manner and using problem solving skills to find a resolution
  • Capacity to manage high caseloads, client accounts and compliance
  • Demonstrated capacity to work effectively with First Nations and CALD colleagues and clients
  • Experience with Home Care Packages (HCP) or Commonwealth Home Support Programme (CHSP)
  • Experience in dementia or palliative care
  • Understanding of Specialised Support Services or Allied Health coordination
